WHAT TO EXPECT AT YOUR FIRST VISIT
COMPLETE NEW PATIENT REGISTRATION
Please complete your New Patient Registration Forms using the link sent to you via text and/or email prior to your initial appointment.
If these are not complete prior to your initial appointment, your appointment may be utilized to complete these forms.
NO MEDICATION MANAGEMENT
We do not provide chronic medication management, either of opiate or non-opiate medication. Initial appointments are for consultation/evaluation only, any recommended treatment will take place on subsequent visits.
We are currently contracted with most insurance carriers. We will do our best to verify coverage and eligibility. All patients are responsible for knowing and understanding your insurance coverage. A copy of your insurance card is required.
Initial consultations for Regenerative Medicine procedures will be billed to regular health insurance. All subsequent treatments are not covered by insurance, however we offer payment plans and 0% interest financing.
PAYMENTS
Payment is expected at the time of visit. This includes co-payments, coinsurance, deductibles or any amount not covered by insurance.
CANCELLATION POLICY
Please contact our office at least 24 hours in advance if you need to cancel or re-schedule your appointment. Failure to do so may result in a NO-SHOW fee on your account ($100/office visits, $150/procedures).
MEDICAL AESTHETICS
Please complete and submit your New Patient Forms prior to your initial treatment.
Pre-treatment directions will be sent to you prior to your treatment.
These treatments are not covered by health insurance, however we offer payment plans and 0% interest financing.

REFERRING PROVIDERS
We offer a multidisciplinary approach to pain management and regenerative medicine for patients with acute and chronic pain.
We do not provide chronic medication management, either of opiate or non-opiate medication.
Referring Providers can expect our office to contact the patient within 24-48 hours of receiving a referral.
